When you decide to purchase a new car, you first decide what is important to you. If mileage and dependability are the important factors, you will search for data-focused more on these factors and less on color options and sound systems.

The same holds true when searching for research evidence to guide your clinical inquiry and professional decisions. Developing a formula for an answerable, researchable question that addresses your need will make the search process much more effective. One such formula is the PICO(T) format.

In this Discussion, you will transform a clinical inquiry into a searchable question in PICO(T) format, so you can search the electronic databases more effectively and efficiently. You will share this PICO(T) question and examine strategies you might use to increase the rigor and effectiveness of a database search on your PICO(T) question.

To Prepare:

· Review the Resources and identify a clinical issue of interest that can form the basis of a clinical inquiry.

· Review the materials offering guidance on using databases, performing keyword searches, and developing PICO(T) questions provided in the Resources.

· Based on the clinical issue of interest and using keywords related to the clinical issue of interest, search at least two different databases in the Walden Library to identify at least four relevant peer-reviewed articles related to your clinical issue of interest. You should not be using systematic reviews for this assignment, select original research articles.

· Review the Resources for guidance and develop a PICO(T) question of interest to you for further study. It is suggested that an Intervention-type PICOT question be developed as these seem to work best for this course.

Post a brief description of your clinical issue of interest ( use matrix sheet ) . This clinical issue will remain the same for the entire course and will be the basis for the development of your PICOT question. Then, post your PICO(T) question, the search terms used, and the names of at least two databases used for your PICO(T) question. Describe your search results in terms of the number of articles returned on original research and how this changed as you added search terms using your Boolean operators. Finally, explain strategies you might make to increase the rigor and effectiveness of a database search on your PICO(T) question. Be specific and provide examples.

Building upon what you have learned thus far in the course, you will take the focus of your clinical inquiry (aka topic of interest) and turn it into a searchable PICO(T) question. In your discussion, you will present your PICO(T) question and address strategies useful to enhance the effectiveness of searching databases for information linked to such.

I often get questions from students regarding the assignment noted in this module, so I wanted to offer some additional insights here. I developed the following in hopes that it can be of assistance.

To develop a PICOT question, it is important that the correct content is connected to each area of the question.

In the past, several students asked about how to address PICOT questions in a specific direction. A variety of focused PICOT questions will be offered here for your consideration:

When considering a question linked to prevention or intervention, one might say:

In adolescents, 13-15 years of age,  how does education by the school nurse compares with groups led by peer mentors in alcohol consumption?

In individuals with a substance use disorder, how does cognitive behavioral therapy compare with life regression therapy in relapse prevention?

When considering a question linked to etiology, one might say:

Are adults, over the age of 25, with a history of alcoholism in their immediate family, compare with those without any immediate family with an alcohol use disorder, at greater risk for developing an alcohol use disorder if social drinking is initiated?

Are adult children of alcoholics (ACOA) who exhibit a genetic predisposition for chemical dependency compare with ACOAs without the genetic predisposition for substance abuse disorders at an increased risk for developing an alcohol abuse disorder>?

When considering a question linked to health prediction, one might say:

In patients who have received an alcohol use disorder diagnosis, how does the use of Declinol compare to not receiving any pharmaceutical intervention when first attempting to stop drinking alcoholic beverages?

In middle-aged females suspected of an alcohol use disorder, are health interviews, as compared to using the “AUDIT” screening instrument,  more accurate in predicting health consequences related to alcohol use?

When considering a question linked to meaning, one might say:

See also  Nursing Research WK 15: You have been asked to be a peer reviewer for a team of nurse researchers...

How do individuals with a diagnosis of an alcohol use disorder, perceive their responsibility for consequences linked to alcohol use before recovery?

· How do individuals raised in a family with a parent with an alcohol use disorder, perceive health issues linked to their own use of alcohol?

By providing these examples, I hope you can see how different dynamics can be applied based on the focus of your work.  Please let me know if you have any questions.

Why you chose this article and/or how it relates to the clinical issue of interest (include a brief explanation of the ethics of research related to your clinical issue of interest)Mental health care is becoming a very emotive healthcare issue today that needs to be mitigated. In this way, it is only possible for the issue to be mitigated if the nurses dealing with people with mental illnesses have sufficient skills and knowledge to handle them. It is because of this that this article is important as it provides information on why nurses working with mental health patients need to be up-skilled.Evidence has indicated that upskilling is very important in enhancing the performance of the employees. In this perspective, health care organizations have the responsibility of retraining their employees such as nurses to ensure that they perform optimally. This article was selected because it provides information on the benefits that organizations accrue by upskilling their nurses.Healthcare assistants play a big role in the management of patients with chronic illnesses such as dementia. In this way, there is a need to have nurses with high skills and knowledge to help reduce the predicaments of such patients. However, it is challenging to provide upskilling training to the nurses working with people with chronic today. The current article was selected because it provides insights on the significance of upskilling in providing high-quality care to patients with chronic illnesses. It also helps to explain the ethical concerns that healthcare assistants face while dealing with patients.Nurses working in the communities are faced with a lot of challenges because they deal with patients with different health care needs. In this way, the nurses need to have enough knowledge and skills to deal with the predicaments of such patients. This article was identified among the many because it provides important insights on why it is important to retrain nurses working in the community.
Brief description of the aims of the research of each peer-reviewed articleThe main aim of this quantitative study was to determine the efficacy of 2-day metabolic workshops that was done to provide nurses with the skills to deliver lifestyle interventions. The current stud aimed to investigate the connection between performance and upskilling. It also explores the benefits of upskilling to the employees and the organization. The study aimed to provide insights on the perspective, role, and experience of healthcare assistants in managing and assessing pain among patients with dementia. It also provides insights on the role of training in improving the management and assessment skills of healthcare assistants. The paper aimed to examine the challenges of training nurses working in the community to commence catheterization for uncomplicated patients in the community.
Brief description of the research methodology used Be sure to identify if the methodology used was qualitativequantitative, or a mixed-methods approach. Be specific. The study used a mixed method study design. Data collection was done using questionnaires that were sent to the study participants. The sampling method that was adopted in the study was random sampling. The quantitative component compares pre and post-measures of attitudes, confidence, knowledge, and perceived barriers of providing metabolic care using a validated tool (M-BACK) and the qualitative component to elicit more details of the needs, expectations, and plans of participants.A qualitative study design using grounded theory-based analysis of 36 interviews revealed that TAWs rely on enhancing their employability as the primary strategy to combat job insecurity.A qualitative method was assumed, using key informant interviews with healthcare assistants caring for people with progressive dementia oncoming the end of life in hospice, nursing home, and severe care settings. Thematic analysis was the analytic method taken to understanding interview data. Data were collected between June 2014 and September 2015Systematic literature review methodology was used to complete the study. Databases searched were MEDLINE, Embase, CINAHL, PsycInfo, and ASSIA from 1980 until March 2015. Studies included were English-language qualitative studies in primary care and community care.
A brief description of the strengths of each of the research methodologies used, including reliability and validity of how the methodology was applied in each of the peer-reviewed articles you selected. The mixed-method was important in this perspective because it allowed the researcher to collect both qualitative and quantitative data that were used to describe and quantify the effectiveness of the program respectively. Similarly, the study design allowed the researcher a wide scope of knowledge to use in the study. The study design was adopted because it allowed the researcher to understand the attitudes of the participants towards upskilling. The validity and the reliability of the study findings were enhanced through an unbiased selection of participants and using an open-ended process which allowed the participants to open up and provide relevant data.A qualitative study design was adopted because it allowed the researcher to understand the attitudes and emotions of the participants towards training and their role as healthcare assistants in helping patients with dementia. The validity and the reliability of the study findings were enhanced through an unbiased selection of participants and using an open-ended process which allowed the participants to open up and provide relevant information. For many years, literature review methodology has been used to investigate problems and provide solutions. The strengths of the methodology are found in the fact that it gives the researcher a wide area to explore already established literature on the topic and synthesize new knowledge. The validity and reliability of the findings are enhanced by the fact that only peer-reviewed studies are used.
General Notes/CommentsThe findings of the study indicate that nurse upskilling is very important as it allows new nurses to gain more knowledge and experience to work with people with mental health problems and provide them with important interventions.Helping TAWs improve employability is important for TAWs, staffing agencies as well as clients. With improved employability, TAWs overcome job insecurity—a disablement to job show, which is of direct significance to the worker and the customer—and also affects the staffing agency’s long-term association with the client. In general, studies have already provided sufficient reasons why healthcare organizations should involve in upskilling programs to increase the number of organizations. The current article, therefore, plays a big role in emphasizing why nurse upskilling is important from a clinical perspective.Studies have shown that community care is at the head of the National Health Service reforms. Role redeployment from registered nurses to health care assistants is growing. Therefore, there is a need to have community nurses with sufficient knowledge and skills to provide the needed care.
